Schengen Visa Form – Page 2 Instructions

  • Employer and Employer's Address and Telephone Number: Enter the name of your employer/company/organization as well as the establishment's address and telephone number. In the case of students, enter the name and address of the educational institution. 
  • Main Purpose of the Journey: Select the relevant box depending on your purpose of visit. Eg: Visiting friends or family, Tourism, Cultural, Business etc. 
  • Member State of Destination: Enter the Schengen country or countries you plan on travelling to 
  • Member State of First Entry: Enter the Schengen country that is your first point of arrival 
  • Number of Entries Requested: Select the relevant box depending on how often you plan on travelling back and forth between your country and the Schengen area 
  • Duration of Intended Stay or Transit: Enter the number of days you plan on staying in the Schengen area 
  • Schengen Visas Issued during the Past Three Years: If this is your first time then select No'. If you have previously travelled to the Schengen area in the last three years select Yes' and enter the validity date of the previous Schengen visa 
  • Fingerprints Collected Previously: Select the relevant option if your fingerprints were collected when applying for a Schengen visa previously 
  • Entry Permit for the Final Country of Destination: If you possess an entry permit for the final country that you plan on visiting, enter the authority it was issued by as well as the duration of its validity 
  • Intended Date of Arrival in the Schengen Area: Enter the date you expect to land in the Schengen area 
  • Intended Date of Departure from the Schengen Area: Enter the date you expect to leave the Schengen area 
  • Surname and First Name of the Inviting Person in the Schengen States: Enter the first and last name of the person inviting you to any of the Schengen countries. If this does not apply, then enter the name of the hotel or accommodation you plan on staying at. Schengen Visa Form – Personal & Travel Details

  • Details of the Contact Person in the Company: If you are applying for a business visa or are travelling on official work, then enter the name, address, telephone number and email ID of the contact person in the organization along with the telephone numbers of the company or organization. 
  • Cost of Travelling and Living during the Applicant's Stay Covered: If you are covering your own travelling and living expenses, select the relevant box along with the relevant means of support such as cash, credit card etc.
  • Personal Data of the Family Member who is an EU Citizen: In the spaces provided, enter your relative's last name, first name, date of birth, nationality and passport or ID number 
  • Family Relationship with an EU Citizen: Select the relevant box stating your relationship with the family member you are visiting. Eg: Spouse, child etc.
  • Place and Date: Enter the city you currently reside in as well as the date of filling in the form
  • Signature: Sign the form, and the case of minors the parents or guardian's signatures are required

How to Apply for a Schengen Visa 

To apply for Schengen visa, you must follow the steps given below:

  • Identify the type of visa you require depending on the German Embassy's or Consulate's jurisdiction, which is covered by individual locations

Cash or a card (Visa, Rupay, and Mastercard are accepted) can be used to pay fees 
  • Go to an application center for a visa 
  • The status of your visa application can be monitored online. To use this service, enter your last name and the Reference Number found on the Visa Application Center's invoice or receipt 
  • Once a decision has been made about your visa application, you may either pick up your passport and documents from the Visa Application Center or pay an extra cost to have your documents returned by courier. 

What Would Happen If the Proper Visa Were Not Obtained? 

Airlines are significantly penalized for carrying passengers who are not authorized to board; thus, they typically won't let you board unless you have the appropriate visa. In the unlikely event that you reach the border; you will be sent back on the next available flight rather than being accepted. Future visa applications will undoubtedly be impacted by the information placed into the Visa Information System. Serious infractions, such using fraudulent documents or lying to border guards, can result in a years-long ban from entering the entire Schengen Area. All the Schengen Area's countries are informed of these restrictions, which are difficult to remove. Although it is possible, the process of appealing if you have been banned or have received a SIS alert is fairly complicated. 

How to Download the Schengen Visa Application Form?

How to Complete the Application for a Schengen Visa 

There are three ways to fill out the form:

  • Online portals: Applicants can fill out the form online and print the resulting PDF in several states, including Germany, France, Italy, and Latvia 
  • Fillable PDF: Before printing, you can enter your information into the official PDF 
  • By hand: The form must be filled out completely in black or blue ink when online facilities are unavailable.

Every application is kept in the Visa Information System (VIS), an EU-wide database that supports border inspections, avoids repeated applications (sometimes known as "visa shopping"), and validates identity. Regulation (EU) 2021/1134 states that personal information, including identity, travel documents, fingerprints, and photos, is kept for a maximum of five years and may be reviewed by border guards and consulates to guarantee security.
